Anne Carle, RN
Anne Carle has been at Cornerstone VNA (CVNA) for seven years, and in the Telehealth Clinical Coordinator position for four years. Anne is dedicated continuously improving CVNA's telehealth program to better patient care and clinical outcomes.
As Telehealth Clinical Coordinator, Anne serves as a triage nurse for clinicians and patients in the field; assessing high-risk patients and scheduling nursing visits when necessary. Anne connects with case managers to recommend changes in patient care plans and coordinates changes with the patients’ referring physicians.
Anne is a long-time advocate of video conferencing, utilizing the tool to introduce new patients to telehealth, build relationships with patients, and provide additional education and training. These virtual visits and interventions have repeatedly prevented unnecessary readmissions and ER visits for Cornerstone VNA patients.
Anne has been recognized repeatedly for her work as Cornerstone's Telehealth Clinical Coordinator having been recognized by Telehealth Honors®. Her expertise is sought after both locally and nationally by healthcare professionals. Anne was a featured speaker at HRS' 2019 Telehealth Advancement Conference and serves on the Mobile Integrated Health Committee at Frisbie Memorial Hospital. Serve on the CVNA Chronic Disease Management Team.
Cornerstone VNA's telehealth program has led to a dramatic decrease in patient readmissions and ER visits, due in large part to Anne's commitment to telehealth and healthcare innovation.
